Abstract

"Statistics and Probability in Forensic Anthropology provides a practical guide for forensic scientists, primarily anthropologists and pathologists, on how to design studies, how to choose and apply statistical approaches, and how to interpret statistical outcomes in the forensic practice. Abstract

"The syndrome X or metabolic syndrome may be defined as a constellation of several interrelated risk factors of metabolic origin which includes centrally distributed obesity, decreased high density lipoprotein cholesterol, elevated triglycerides, elevated blood pressure and hyperglycemia. Persons with syndrome X are at twice the risk of developing cardiovascular disease, and are at about five-fold the risk of type 2 diabetes mellitus as compare to persons with this syndrome. There are considerable evidences of important ethnic differences in the prevalence of syndrome X, its components and sequelae. The prevalence varied between ethnic groups even within the same geographical location. Apart from multifactorial (genetic vs. environmental) nature, there are intriguing metabolic and anthropometric differences found across different ethnic groups. The estimates vary by ethnicity, for instance, high prevalence in non-European groups such as Black African-Caribbeans, Hispanics, indigenous populations of North America, South Asians, and the Western Pacific Region and lower prevalence in Chinese populations, and European-White. Public health strategies that are well known to be important for chronic disease prevention in general can substantially reduce the prevalence of syndrome X. Population-specific studies will be important in identifying subgroups for which syndrome X is a health issue and for which disease management strategies are needed. Simultaneously, studies to identify different biomarkers associated with syndrome X that are linked with the development of specific chronic diseases, such as CVD and diabetes, will significantly enhance the early detection of these diseases. Existing preventions strategies, if implemented in population subgroups at highest risk, may have a substantial effect on reducing these trends. The prevalence of syndrome X increases with advancing age suggests that the efforts to increase awareness of prevention strategies must begin early. Intensive treatment and effective targeting efforts would help in avoiding future sequelae of syndrome X across different ethnicity. This book will be an assemblage of similarities and differences in the risk of syndrome X and related disorders among different ethnic groups worldwide, and reviews potential mechanisms for ethnic differences. It would certain help in translational research for early intervention and better prevention of syndrome X and its associated public health burden worldwide"--

[Italiano]: Il farmaco, nella sua accezione più ampia e generale, può essere ritenuto un bene sociale, la cui valenza simbolica e curativa varia in relazione alla dimensione ambientale e culturale nel quale si inserisce. In tal senso, le prescrizioni farmaceutiche rappresentano un indicatore privilegiato per la conoscenza del sistema salute di un determinato territorio, poiché costituiscono un punto di intersezione ideale tra la prospettiva medica e quella di mercato. Siffatte considerazioni hanno sollecitato l’elaborazione di questo Report che si pone, come obiettivi dichiarati, quello di essere uno strumento utile alla pianificazione di interventi di sanità pubblica, quanto quello di svolgere analisi approfondite sulle caratteristiche dei soggetti che usano i farmaci e sulle modalità di trattamento degli stessi, permettendo studi di appropriatezza prescrittiva su specifiche aree di rilevanza clinica e su specifiche coorti di soggetti. “PharmaCaRe Report 2018” è stato realizzato dal CIRFF (Centro Interdipartimentale di Ricerca in Farmacoeconomia e Farmacoutilizzazione) dell’Università degli Studi di Napoli Federico II, in collaborazione con la Direzione Generale della Tutela della Salute della Regione Campania, per delineare un quadro dettagliato circa il consumo e la prescrizione dei farmaci in Campania nel 2018. Questo Report intende infatti fornire una fotografia dettagliata dell’utilizzo che, in Campania, viene fatto dei farmaci in termini di spesa, volumi e tipologia. Le analisi dei dati prodotte offrono spunti importanti per correlare la prevalenza delle patologie nel territorio con il corrispondente utilizzo dei farmaci e suggeriscono un’interpretazione dei principali fattori che influenzano la variabilità nella prescrizione. La disponibilità di una banca dati che copre una popolazione assistibile di circa sei milioni di abitanti è d’altronde un potente strumento di ricerca per studiare gli effetti dell’utilizzo dei farmaci in condizioni di Real-World. La conoscenza delle dinamiche prescrittive, in termini qualitativi (appropriatezza d’uso), oltre che quantitativi (volumi di utilizzo) è la condizione necessaria per inquadrare in un contesto razionale la politica del farmaco, anche sotto il profilo della valutazione degli effetti degli interventi che il mercato, le normative o la cultura del farmaco sviluppano nel tempo. Per tali ragioni, “PharmaCaRe Report 2018” rappresenta un utile quanto prezioso supporto ai decisori per individuare strategie volte a ottimizzare l’allocazione delle risorse, nonché migliorare i percorsi di cura attraverso un monitoraggio costante, la promozione di più elevati standard di cura e l’uso sicuro, efficiente ed efficace dei farmaci ./[English]: In its broadest and most general sense, the drug can be considered a public resource, whose symbolic and curative value varies in relation to the environmental and cultural dimension in which it is embedded. In this sense, pharmaceutical prescriptions represent a privileged indicator for the knowledge of the health system of a given territory, since they constitute an ideal intersection point between the medical and the market perspective. Such considerations prompted the preparation of this Report. “PharmaCaRe Report 2018” has been produced by CIRFF (Centro Interdipartimentale di Ricerca in Farmacoeconomia e Farmacoutilizzazione) of the Federico II University of Naples, in collaboration with the Directorate-General for Health Protection of the Campania Region, to provide a detailed overview of the pharmaceutical consumption and prescriptions in Campania in 2018. This Report aims to provide a detailed picture of the use of medicines in the general population in Campania, in terms of expenditure, volumes and type. The analyses of the data produced offer important clues for correlating the prevalence of diseases in this area with the respective use of medicines and suggest an interpretation of the main factors influencing prescriptions' variability. The availability of a database covering a patient population of around six million is a powerful research tool for studying the effects of drug use in Real-World conditions. Knowledge of the dynamics of prescription, in qualitative terms (appropriateness of use), as well as quantitative (volumes of use) is the necessary condition to frame the drug policy in a rational context, also in terms of evaluating the effects of the interventions that the market, regulations or drug culture develop over time. For these reasons, “PharmaCaRe Report 2018” represents a useful and valuable tool for political decision-makers in identifying strategies aimed at optimizing the allocation of resources, as well as improving care pathways through constant monitoring, the promotion of higher standards of care and safe, efficient and effective use of drugs.

Abstract

Die Qualität der Versorgung, ihre Sicherung, Verbesserung und Incentivierung sind das erklärte Ziel der Gesetzgebung im Gesundheitswesen der letzten Jahre. Der Qualitätsmonitor widmet sich mit der aktuellen Ausgabe der Notfallversorgung, der Transparenzmängel und Steuerungsdefizite attestiert werden. Für die Diskussion über die Qualität und die Weiterentwicklung der prästationären und stationären Notfallversorgung liefert der Qualitätsmonitor 2020 weitere Impulse. Das von Gesundheitsstadt Berlin, der Initiative Qualitätsmedizin (IQM) und dem Wissenschaftlichen Institut der AOK (WIdO) herausgegebene Werk greift wiederholt relevante Qualitätsthemen aus stationärer und sektorenübergreifender Versorgung auf. Die Fachbeiträge zum Schwerpunkt Notfallversorgung stellen den Stand der politischen Reformdiskussion dar und betrachten für lebensbedrohliche Indikationen die stationäre Notfallversorgung, auch unter Berücksichtigung der bisherigen Erkenntnisse aus der COVID-19-Pandemie. Strukturen und Prozesse im Rettungswesen, regionale Modellprojekte sowie der Stand der Digitalisierung werden diskutiert und Verbesserungsvorschläge unterbreitet. Eine weitere Sektion zeigt Eckdaten zur Versorgungsstruktur und Qualitätsdaten der deutschen Krankenhäuser für ausgewählte Notfallindikationen der stationären Versorgung. Mit seinen Fachbeiträgen und bundesweiten Qualitätsdaten unterstützt der Qualitätsmonitor 2020 fundiert die Qualitätssicherungsdebatte in Deutschland. Den gesundheitspolitischen Akteuren und Anbietern, die mit der Schaffung und Umsetzung gesetzlicher Qualitätsvorgaben befasst sind oder diese mitgestalten wollen, werden relevante Ansätze aufgezeigt und Anregungen zur Weiterentwicklung und Verbesserung der Versorgungsqualität gegeben.

Abstract

Evidenz beschreibt etwas, das unmittelbar einleuchtet. Wissenschaftliche Abbildungen erheben zwar den Anspruch, evident zu sein, aber ihre Evidenz ist nicht natürlich. Sie ist nur mittelbar für diejenigen einleuchtend, die sie lesen können. Das gilt auch für Bilder in der medizinischen Diagnostik: Im Visualisierungsprozess diagnostischer Abbildungen werden zum einen Strukturen und Zusammenhänge sichtbar gemacht, die zuvor unsichtbar waren. Zum anderen muss über Lese- und Denkprozesse die Evidenz in das Bild implementiert werden. Allerdings sind diese Bilder primär durch Mehrdeutigkeit und Unbestimmtheit geprägt, was Strategien der Evidenzproduktion erfordert. Hierzu gehören der Einsatz geschulter Zeichner am Mikroskop, die Produktion von Diagrammen auf Basis von Messwerten, der versierte Einsatz von Endoskopen sowie die physikalisch-chemische Erzeugung von Radiogrammen zur Sichtbarmachung des Körperinneren. Die Evidenz diagnostischer Abbildungen unterliegt also immer bestimmten Verfahren und Strategien der Evidenzzuschreibung. Sie bleibt eine nützliche Fiktion, die überaus vielfältig und immer kontextabhängig ist. Heiner Fangerau und Michael Martin sprechen deshalb statt von Evidenz im Singular von den Evidenzen diagnostischer Bilder im Plural.

Abstract

Myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) is a severe chronic health condition that is often misunderstood or ignored by health establishments. The lack of definitive diagnostic markers to separate ME/CFS patients from the healthy population as well as from other chronic disorders is problematic for both health professionals and researchers. A consortium of Australian researchers gathered to systematically understand ME/CFS, ranging from a deep analysis of clinical and pathology data to metabolomic profiles and the investigation of mitochondrial function. From this broad collaboration, a number of compelling insights have arisen that may form the basis of specific serum, blood, and/or urinary biomarkers of ME/CFS. This Special Edition reports on a conference centred on these biomedical discoveries, with other contributions, with a translation focus for predictive markers for ME/CFS diagnosis. By supporting health professionals with developments in diagnostics for this condition, the patients and their families will hopefully benefit from an improved recognition of the biomedical underpinnings of the condition and will be better able to access the care that is urgently required. This Special Edition contains a mix of speaker submissions and other accepted manuscripts that contributed to our objective of advancing biomedical insights to enable the accurate diagnosis of ME/CFS.

Abstract

In Unani Medicine in the Making, Kira Schmidt Stiedenroth examines the contemporary institutions and practices of Graeco-Islamic healing in India. Drawing on interviews with practitioners, clinical observations, and Urdu sources, the book focuses on Unani's multiplicity, scrutinizing apparent tensions between the understanding of Unani as a system of medicine and its multiple enactments as Islamic medicine, medical science, or alternative medicine. Ethnographic details provide vivid descriptions of the current practices of Unani in India and invite readers to rethink the idea that humoral medicine is incommensurable with modern science. Ultimately, the book also discusses the relationship of Unani with Muslim communities, examining the growing practice of Prophetic Medicine in Urban India and the increasing representation of Unani as Islamic Medicine.
